For more than a decade, this wasn't just something I taught—it was my life's work.

Before I became known as Savvy Business Chick, before I mentored hundreds of aspiring agency owners across the country, my very first Home Health Care agency specialized almost exclusively in caring for medically fragile infants, children, teenagers, and young adults.

For the first 11 years, pediatric skilled nursing was our primary focus.

It became more than a business.

It became our mission.

Where It All Started...

Many people have never heard the name Katie Beckett.

But if you own—or plan to own—a pediatric Home Health Care agency, you owe a great deal to her family.

In 1978, Katie Beckett was born with significant medical complications that required long-term ventilator support. Although her physicians determined she could safely receive care at home, Medicaid policy at the time would only pay for her care if she remained in the hospital.

The government was willing to spend far more money to keep Katie in a hospital than it would cost to care for her safely in her own bedroom.

Katie's parents fought tirelessly to change that.

Their advocacy reached President Ronald Reagan, who directed federal officials to create what would become known as the Katie Beckett Waiver (also referred to in many states as the TEFRA option).

For the first time, families could qualify their medically fragile child for Medicaid based on the child's disability—not solely on the parents' income—and many states began expanding Home and Community-Based Services (HCBS) so children could receive care where they belonged...

When I started my agency in Maryland, I wasn't a nurse.

I had an incredible nurse mentor who helped me understand the clinical side of pediatric care.

But my greatest teachers were the families themselves.

Every day I met parents who had become nurses out of necessity.

Many had spent months inside children's hospitals learning how to suction tracheostomies, administer medications through feeding tubes, monitor oxygen, recognize seizures, and respond to medical emergencies because there simply wasn't anyone else available to care for their child.

Our agency became their support system.

We cared for children with:

Many hadn't had a date night in years.

Home health care wasn't just providing nursing.

It was restoring hope.

It was restoring marriages.

It was restoring families.

Those experiences helped grow my agency into a multi-million-dollar pediatric Home Health Care company—but more importantly, they shaped the mentor I am today.

The need has not disappeared.

If anything, it continues to grow.

1. Maryland

One of the nation's most respected programs through the Model Waiver for Medically Fragile Children. Maryland remains a premier state for agencies interested in pediatric skilled nursing and medically complex care.

2. Illinois

Illinois continues to operate one of the country's strongest Medically Fragile and Technology Dependent Waiver programs, supporting children who require extensive nursing and medical services in the home.

3. Texas

With one of the nation's largest pediatric populations and the Medically Dependent Children Program (MDCP), Texas offers tremendous opportunity for agencies specializing in pediatric services.

4. Oregon

Oregon has developed multiple waiver pathways for medically involved children, making it an innovative state for pediatric Home and Community-Based Services.

5. Utah

Utah's Medically Complex Children's Waiver demonstrates a strong commitment to keeping children at home with their families instead of institutional settings.

6. Virginia

Virginia continues to provide robust services for medically complex children through its Commonwealth Coordinated Care Plus (CCC Plus) Waiver, making it an attractive state for pediatric providers.

7. Wisconsin

The Children's Long-Term Support Waiver continues to provide extensive supports for children with developmental disabilities and significant medical needs.

New York's Children's Waiver offers comprehensive services that help medically complex children remain safely at home while supporting their families.

Arkansas deserves recognition for maintaining one of the country's strongest Katie Beckett/TEFRA-style Medicaid eligibility pathways, helping many families gain access to essential healthcare services for their children.

"What About Georgia?"

One question I receive frequently is:

"Why isn't Georgia on this list?"

Georgia absolutely deserves recognition because it was one of the first states to implement the Katie Beckett eligibility pathway after the federal policy changes inspired by Katie's story.

However, this article isn't ranking states based on historical significance.

It's ranking states based on today's combination of pediatric opportunity, specialized waiver programs, medically fragile populations, and the overall environment for agencies serving these children.

Georgia remains an important state and continues to help many families. It simply isn't one of the strongest overall opportunities in 2026 when evaluating these factors together.
